Recent reports highlight training deficiencies across multiple sectors. A Court of Inquiry in New Zealand found that the crew of the navy ship HMNZS Manawanui lacked sufficient training and experience, contributing to its 2024 loss. In the UK, a UCL study reported that only 20% of senior NHS leaders had formal management training in the past five years. Separately, an EHS Today survey found that 58% of workers indicated that additional training would improve workplace safety. 

These examples illustrate consistent gaps in essential training, affecting operational effectiveness, decision-making, and safety in both public and private sectors.
